Older Adults Homebased Care Re-design

Edinburgh Health and Social Care Partnership are about to embark on an exciting journey to transform the way homebased services for older adults (aged 65 years and over) are delivered.

Would your organisation have the ambition and commitment to develop and deliver a shared vision with Edinburgh Health and Social Care Partnership, and other partners for these services – creating a ‘One Edinburgh’ approach to homebased support?

Edinburgh Health and Social Care Partnership is setting an ambitious direction of travel, which seeks to build on all its strengths, identifying areas where it needs to do things differently and then work together with its partners to provide the best possible homebased support outcomes for the citizens of Edinburgh. It believes that effective services must be designed with and for people and communities, moving away from transactional behaviours and processes and encouraging greater partnership working.
